Referral Admission

Our intake coordinator handles all admission requests. Once a request is made regarding admission, we will review within 24 hours. At that time, we will determine if the child or adolescent meets the criteria for our residential services.

Our clinical team will review the adolescent’s information and will grant approval to proceed with placement at the facility. At that time, we will request all required documentation and coordinate a time and date for admission.

Upon admissions you will need to:

  • Must be transported by guardian or assigned DSS case manager
    • If being transported by a DSS transporter, we will need a letter on DSS letterhead from the assigned case manager giving permission for the transporter to sign all consents prior to arrival
    • Transporter must show ID to confirm
  • Please make sure youth come with at least a 2-week supply of medications and updated MAR (MAR MUST match medications). It is the guardian’s responsibility to fill any prescriptions before the youth is admitted to AHS. We do NOT have a pharmacy on campus and do not have immediate access to prescribed medications. We need enough time to order and have the medications mailed to us.
  • Please be prepared to fill out the Visitation and Contact Plan
    • If being transported by anyone other than Guardian, the Guardian is responsible for coordinating this to be completed prior to admission
